Required Documents: We require all patients to complete each of the following forms in order to qualify for assistance. Paperwork must be received within the required timeframe . We also require an original picture of the patient and a brief story for our files. If more than one flight will be needed, please make copies of the blank forms to use for future flight requests.

Miracle Flights for Kids® is committed to providing our patients with optimum conditions for comfort and dignity. In order to accomplish this, we have set up the following criteria to aid us in accepting those patients who will benefit to the greatest extent from our charitible flights:
We require at least 10 business days for domestic and no fewer than 21 business days for international flight requests.
Must be able to board the aircraft unassisted. No stretchers. Wheelchairs are permitted.
May be a blood or organ donor.
May be accompanied if space is available on private aircraft.
Must not be on any life support systems.
In addition to the Doctor Certification Form, must provide a doctor's statement (on their letterhead) confirming the medical diagnosis/condition, the reason travel is required, and the date of appointment(s).
Must be going to or from a recognized treatment center.
Must arrange ground transportation for themselves and family members to and from the airport.
Must provide at least 10 business days advance notice of flight requirements, including exact date of appointment. Must be flexible with flight request. When commercial tickets are purchased, this advance planning permits us to obtain the best fares and better utilize our financial resources to help more kids.
Must not smoke in or around aircraft.
Must arrive at least 2 hours prior to the scheduled departure.
Must sign waivers prior to boarding private aircraft.
Must be aware that mechanical problems, weather conditions, acts of God, or other factors may result in flight cancellations or delays.
Must not be on any medications that may cause adverse effects at altitude.

Miracle Flights for Kids® is unable to guarantee nonstop or direct flights. We do our best to schedule comfortable flights for the families we assist, however, as a charity, we must always purchase the least expensive flight available.
Miracle Flights for Kids® volunteer pilots reserve the right to use reasonable judgment when accepting or declining a flight. A pilot may decline a flight if he/she judges that the individual mission might jeopardize the welfare and safety of the patient and/or crew.
